Remote Patient Monitoring Made Simple
The Heart Network's 24/7 Digital Health Command Center and Home-Based Cardiac Rehab programs give affiliated practices the infrastructure to extend care beyond the visit, improving outcomes for your highest-risk patients and generating Medicare-reimbursable revenue in the process.
A Ready-to-Deploy Virtual Care Infrastructure
What’s Included
Everything Your Practice Needs to Extend Care Beyond the Visit
Remote Patient Monitoring (RPM)
Daily home monitoring via blood pressure machine, weight scale, and pulse ox, with readings transmitted automatically and reviewed by a dedicated cardiac monitoring team every day. Medicare-reimbursable and deployable without adding clinical burden to your staff.
Chronic Care Management (CCM)
Structured care management for patients with two or more chronic conditions, including personalized care planning, monthly check-ins, medication support, and care coordination across providers. Medicare-reimbursable with established billing codes.
Principal Care Management (PCM)
Non-face-to-face care management for patients with a single high-risk complex chronic condition, including specialized care planning, medication management, and 24/7 care team access. Minimum 30 minutes per month. Medicare-reimbursable.
MCOT & CIED Surveillance
Mobile Cardiac Outpatient Telemetry and Cardiac Implantable Electronic Device surveillance, with continuous real-time cardiac rhythm monitoring and device management for your highest-risk patients, reviewed by a dedicated clinical monitoring team.
Home-Based Cardiac Rehab
An evidence-based alternative to traditional facility-based cardiac rehabilitation delivered in the patient's home through a structured, supervised program. Expands access for patients who can't or won't attend facility-based rehab, improving outcomes and reducing readmissions.
Dedicated Monitoring Team & Platform
A fully integrated virtual care platform with a dedicated clinical monitoring team that reviews patient data, triggers proactive outreach, and serves as a direct point of contact for enrolled patients without adding headcount to your practice.

What Our Command Center & Virtual Care Delivers for Your Practice
Increased Revenue Opportunities
Remote patient monitoring, chronic care management, MCOT, CIED surveillance, and Home-Based Cardiac Rehab are established Medicare programs with clear reimbursement codes, and most independent cardiovascular practices aren't capturing that revenue. Not because their patients don't qualify, but because deploying the programs requires infrastructure they don't have. The Heart Network's command center changes that.
Better Outcomes for Patients and VBC
For patients managing heart failure, complex arrhythmias, or coronary artery disease, daily vital sign monitoring and continuous rhythm surveillance create a clinical picture that quarterly office visits simply can't provide. MCOT and CIED surveillance give your care team real-time visibility into your highest-risk patients, enabling earlier intervention, reducing hospitalizations, and improving the quality metrics that drive value-based care performance.
Proven Infrastructure Already in Place
The barrier to deploying virtual care programs in an independent cardiovascular practice has historically been operational rather than clinical. Equipment logistics, patient enrollment, daily monitoring, proactive outreach, billing, and compliance all require infrastructure most practices can't build independently. The Heart Network's 24/7 Digital Health Command Center handles all of it.
How It Works
From enrollment to active program deployment in four steps.
Discovery Call
We learn about your current patient population, your highest-risk cohorts, and which programs — RPM, CCM, MCOT, CIED surveillance, or Home-Based Cardiac Rehab — are the best fit for your practice. No commitment required.
Practice & Patient Assessment
Our team reviews your eligible patient population and identifies which Medicare programs they qualify for. You get a clear picture of the revenue opportunity: how many patients qualify, what the reimbursement looks like, and what deployment would mean for your practice specifically.
Program Setup & Patient Enrollment
We configure the virtual care platform, handle equipment logistics, and manage patient enrollment. Your clinical team is trained on workflows and escalation protocols. Most practices are live within 30–60 days of agreement.
Go-Live & Ongoing Support
The dedicated monitoring team reviews patient data and manages proactive outreach when readings fall outside parameters. Your practice receives regular performance reporting with ongoing support to help you expand the program as your patient population grows.

Questions We Hear Most About Virtual Care Programs
If you have something specific you'd like to ask, we'd love to have a conversation.
RPM is appropriate for patients requiring daily monitoring of chronic conditions. Heart failure, hypertension, and complex arrhythmias are among the most common cardiovascular indications. CCM is for patients with two or more chronic conditions. PCM is for patients with a single high-risk complex chronic condition. MCOT is for patients with suspected but undetected arrhythmias requiring continuous rhythm monitoring. CIED surveillance is for patients with implantable cardiac devices including pacemakers, ICDs, and loop recorders. Home-Based Cardiac Rehab is for patients who qualify for cardiac rehabilitation but cannot or prefer not to attend a facility-based program. Eligibility is reviewed during the practice assessment phase.
Enrolled patients receive home monitoring equipment, typically a blood pressure machine, weight scale, and pulse ox. Readings transmit automatically to the monitoring team, who review them daily. Patients can also communicate with their care coordinator through a dedicated mobile app. The experience is designed to be simple for patients regardless of their comfort with technology.
Your physicians authorize patient enrollment and review escalated alerts when the monitoring team identifies a concern. Beyond that, the command center infrastructure handles daily monitoring, outreach, and care coordination. The goal is to extend your practice's care delivery without adding meaningful burden to your clinical staff.
RPM, CCM, PCM, MCOT, and CIED surveillance are established Medicare programs with clear reimbursement codes. Home-Based Cardiac Rehab is also Medicare-reimbursable. The specific billing structure and your practice's responsibilities are reviewed in detail during onboarding.
Yes. Most practices start with RPM or MCOT, which have the most straightforward deployment, and add additional programs as the infrastructure and workflows are established. There is no requirement to deploy all programs simultaneously.
